Van Horn Rural Health Clinic

The Van Horn Rural Health Clinic is located in a newly renovated area on the south end of the hospital building. It includes a spacious waiting room and a special “pint-sized” pediatric waiting area designed with the comfort of patients and their families in mind. The 8,000 square foot facility includes 10 exam rooms. Two exam rooms are equipped with electronic tables that can be raised or lowered to aid patients with mobility concerns.  

The clinic has a separate entrance but is connected to the hospital as well. Located off the clinic entrance is Crane Pharmacy.

The clinic is staffed with Medical Providers who are devoted to keeping the whole family healthy.  

Management and Treatment of Chronic Medical ConditionsCulberson Waiting room

Effective management and treatment of various chronic medical conditions is important to maintaining a healthy, active lifestyle, and the prevention and management of complications. Some of the chronic medical conditions managed by your healthcare team at Van Horn Rural Health Clinic  include:

Diagnostic and Screening Tests

Van Horn Rural Health Clinic offers a wide range of diagnostic tests to assist with the development of your wellness or treatment plan.  Services may be provided in the clinic or Culberson Hospital.  

Medicare Wellness Visits

If you are covered by Medicare, you are eligible for a free Annual Medicare Wellness Visit to talk with your provider about ways to improve your health and prevent disease.  The Annual Wellness Visit is not the same as an annual physical exam, but it does provide patients with the same opportunity to talk with their provider about health concerns and to develop a plan in partnership with those providers to focus on health, rather than illness. 

Some of the services covered during an Annual Wellness Visit include:  routine measurements such as weight, height and blood pressure; review of your individual and family medical history, review of your current medications, discussion of the care you receive from other providers, and review of your functional ability and level of safety.  Based on this review, you and your provider will develop a health promotion and disease prevention plan based on your specific needs.

After your initial wellness visit, you are eligible for a follow-up or subsequent wellness visit every 12 months.  These visits are not subject to copayments to deductibles, so there is no cost to the individual.  Call Coleman Medical Associates at 325-625-3533 to schedule your Annual Wellness Visit.

David J. Cummings, M.D.

David J. Cummings, M.D., became a member of the provider team at Van Horn Rural Health Clinic and Culberson Hospital in 2019. He completed a Family Medicine Rural Residency program at the University of Nevada – Las Vegas School of Medicine, and received his medical degree from the Paul L. Foster School of Medicine at Texas Tech University Health Sciences Center in El Paso. Dr. Cummings also earned undergraduate degrees in microbiology and health information technology at the University of Texas, Austin, and an Associate of Arts degree from Temple College in Temple. He maintains active certifications in Basic Life Support, Pediatric Advanced Life Support, Advanced Cardiovascular Life Support and Advanced Life Support in Obstetrics.

Gilda Morales, DC, MSN, ANP-C

Gilda Morales, DC, MSN, ANP-C, began working as a nurse practitioner in the Van Horn Rural Health Clinic in 2015. She is a Van Horn native who pursued her undergraduate and graduate education at Sul Ross State University, Alpine, Texas, and her Doctor of Chiropractic Medicine at Parker College of Chiropractic, Dallas, Texas. As a chiropractor, she worked for a medical clinic in Dallas, Texas, beginning in 1997, and later set up her own practice in Garland, Texas. Morales returned to Van Horn in 2006 where she established Southwest Wellness and Fitness Center before obtaining her Bachelors in Nursing from Western Governors University, Austin, Texas and her Advance Practice Nursing degree from South University, Savannah, Georgia.

Kayla Engle, FNP

Kayla started her nursing career at Culberson Hospital in 2015 after graduating from nursing school at UTEP. While working full-time she continued her education and gained certification to become and Family Nurse Practitioner. She is able to see patients of all ages and especially loves treating children. She and her family live near Van Horn and they love being a part of Van Horn's tight-knit community.

Rodett Osorio, PA

Rodett Osorio, PA-C, brings a strong background in health care to the clinic and hospital. After receiving a Bachelor of Science degree in Clinical Laboratory Science, Rodett practiced in the laboratory setting for over seven years, first as a medical technologist at Saint Elizabeth Regional Medical Center in Lincoln, Nebraska, and later as a laboratory supervisor at McAllen Medical Center in McAllen, Texas. He subsequently returned to Union College where he earned a Master’s Degree in Physician Assistant Studies.
